Take Care Clinics^SM provide a wide range of healthcare services for patients 18 months and older. Ages for specific vaccinations and other services may vary.

Vaccines: Flu (Seasonal and Preservative Free)ages 2+; GARDASIL® series (HPV)ages 11-26; Hepatitis A seriesages 7+; Hepatitis B seriesages 7+; Meningitisages 11-55; MMR (Measles, Mumps, Rubella)ages 7+; PPV (Pneumonia)ages 65+, ages 19 - 64 who smoke or have asthma, ages 7+ with long-term health conditions; Tdap (Tetanus, Diphtheria, Pertussis)ages 10+; Td (Tetanus Booster)ages 7+; Varicella series (Chicken Pox)*ages 7+; Zostavax (Shingles)ages 50+; Check vaccine availability at a location near you.; *The Varicella (Chicken Pox) vaccine may need to be ordered upon request. Physicals*: Camp Physicals; School Physicals; Sports Physicals**; Additional physicals vary by area; *Certain exceptions may apply. Patients who have had a physical at Take Care Clinic within the past year and only need an additional form completed will be charged $15. Patients who need a physical completed for more than one program will be charged an additional $15 per program.; **High School level, pre-participation sports physicals are not available in Kansas.

Wellness Services: Blood Pressure Screening; Health Screening with Health Risk Assessment†ages 18-65; Health Screening†ages 18+; Influenza A and B Testing*; Medicare Wellness Visits; PPD/Tuberculosis Testing; *Cannot be purchased separately; may only be given as part of a paid exam.
