Don't See Your Insurance Plan Listed?
Take Care Clinic's goal is to work with as many insurance plans as possible. If you'd like us to contact your insurance provider on your behalf and ask them to cover your visit to Take Care ClinicSM, contact us. Or call the number listed on your ID card and ask your insurance company to contract with Take Care Clinic.
If your insurance company has not yet contracted with Take Care Clinic, or if you are uninsured, payment is required when services are provided.
Take Care Clinic accepts cash, credit, or debit cards, but no personal checks. Your Take Care Clinic Nurse Practitioner or Physicians Assistant will give you a receipt with all the information you need to submit to your insurer for reimbursement against out-of-network benefits, or to your flexible spending account, health reimbursement account, or health savings account.
